All-on-6 dental implants replace a full arch of missing or failing teeth with a fixed bridge held in place by six titanium implants. The implants act as new tooth roots, anchoring securely into your jaw so your teeth stay put when you eat, talk and smile.

The two front implants are usually placed vertically, while the rear implants are angled to make the most of the bone you have and to sit clear of the sinuses and nerves. That angle is generally less steep than with All-on-4, because the extra implants share the load more evenly. On top of the implants sit multi-unit abutments. These are the connecting platform your bridge screws onto, and they correct for the angle of the implants so your final teeth line up naturally.

If you have been weighing up All-on-4 vs All-on-6, you are not alone. It is one of the most common questions we hear, and it is an important one to get right.

The two treatments have plenty in common. Both are fixed, full-arch solutions built on titanium implants; both are designed to look and feel like natural teeth, and both can provide a set of fixed temporary teeth around the time of surgery.

The difference comes down to the number of implants. All-on-6 uses two more implants than All-on-4. Those extra supports spread your bite force more evenly, shorten the unsupported spans within the bridge, and build in more redundancy across the arch.

There is a trade-off, though. All-on-6 generally needs more jaw bone to work with, because it has to accommodate six implants. All-on-4 was designed around angled rear implants that can suit patients with less available bone, and it often avoids the need for grafting.

Neither option is universally better than the other. Full-arch treatment is a significant step, so it helps to know who is looking after you. Dr Paul Wilson is an Oral Implantologist who has spent more than 40 years in dentistry. Originally from Perth, he graduated from the University of Western Australia in 1980 before training and practising in some of the UK’s most respected settings, including Oxford, Knightsbridge and Harley Street.

He went on to complete postgraduate study in advanced restorative dentistry, dental implants and dental material science at the Eastman Dental Institute, UCL. He is a long-standing member of the ITI (International Team for Implantology), along with the RCS Eng., the ADI and CGDent Eng. The All-on-6 Process at Keys Dental

During your consultation, your dentist will carry out a thorough assessment to determine whether this treatment is right for you.
Your assessment may include:

Consultation and 3D planning.

You meet with Dr Wilson and have a 3D CBCT scan so he can assess your jawbone and plan your implant positions precisely. Your treatment plan and costs are confirmed upfront, so you know where you stand before anything begins.

Implant placement.

Six titanium implants are placed under local anaesthetic, with sedation available if you would prefer it. The rear implants are angled where needed to make the most of your available bone and to stay clear of nerves and sinuses.

Multi-unit abutments and temporary bridge

Multi-unit abutments are fitted to the implants to act as the platform for your bridge. A fixed temporary bridge is then fitted on the day of your surgery, so you leave the clinic with a functional set of teeth while your implants heal.

Healing and integration.

Over the next three to six months, the implants fuse with your jawbone in a process called osseointegration. We keep an eye on your progress with monitored review appointments during this time.

Final bridge placement

Once your implants have integrated, your permanent bridge, for example in zirconia, is screwed onto the abutments and matched to your bite and the look you want. Because it attaches to the abutments, the bridge can be serviced in future without disturbing the implants themselves.
